I create Custom Categories and add IP addresses
but if checked "Block access to web sites using ip address"
sites is blocked.

If you absolutely need to access remote websites through the proxy using IP addresses (this is really uncommon), you should not check Block access to web sites using ip address.
If you have only one website with IP, you can add it to the Sites without proxy tab.
